A case of Ischemic Gastropathy Mimicking Linitis Plastica

Abstract

|
UGIS findings of a 69-year-old female with septic shock and acute abdomen showed irregular luminal narrowing with rigidity from the prepyloric antrum to the high body of the stomach, especially along the greater curvature from the antrum to the lower body, mimicking linitis plastica. CT findings were diffuse wall thickening of gastric body and antrum, intramural low attenuation zone and US finding was diffuse and even wall thickening of gastric body and antrum.
Macroscopic findings of the resected stomach were a large ulcer at the greater curvature from the antrum to the lower body of the stomach and severe submucosal edema. Microscpic findings of the ulcer showed healing state with vasculitis obstructing medium-sized arteries adjacent to the ulcer, and those of the remained stomach showed severe submucosal edema. On the basis of the pathologic findings, diffuse and irregular luminal narrowing with rigidity of the stomach mimicking linitis plastica on UGIS, and diffuse and even wall thickening on US and CT could be explained as gastric wall thickening due to severe submucosal edema. Conclusively, we believe that ischemic gastropathy should be included in the differential diagnosis of linitis plastica on UGIS.
